Casey L. Hogan, OD, FAAO, FSLS, Dipl. AAO
Casey L. Hogan, OD, FAAO, FSLS, Dipl. AAO received her Doctor of Optometry from the Illinois College of Optometry (ICO), after which she went on to complete a primary care residency at ICO, and continued fellowship training at the prestigious Boston Medical Center. She served on faculty as Assistant Professor for over ten years at ICO, and also served as adjunct faculty for the Cornea and Contact Lens residency program.

Her passion lies in ocular surface disease and specialty contact lens care. She recently earned the distinct honor of becoming a diplomate of the American Academy of Optometry and is the immediate past Diplomate Chair of the Anterior Segment Section. She is a consultant within the contact lens industry, has served on pharmaceutical advisory boards, and is currently serving as President of the Illinois College of Optometry Alumni Association Board of Directors.

Dr. Hogan is a Fellow of the American Academy of Optometry, a Fellow of the Scleral Lens Education Society, and a member of the American Optometric Foundation President’s Circle.

She is a member of numerous professional organizations and is a past President of Special Olympics Chicago.

In her free time, she enjoys studying piano, spending time with her family and dogs.

Tiffany Polanek, OD
Tiffany Polanek, O.D. received her Doctor of Optometry from the Michigan College of Optometry. She earned her Bachelor of Arts from Moorhead State University.

Dr. Polanek continued her training at the Illinois College of Optometry, completing the primary care and ocular disease residency program in 2001. She has provided optometric care in a variety of different settings, including 15 years of teaching at the Illinois Eye Institute as an Associate Clinical Instructor in both the Primary Care Clinic and in the Cornea and Contact Lens Center for Clinical Excellence. While there, she provided eye care to patients, and clinical education to optometry students.

Having extensive expertise in both adult and pediatric primary eye care, contact lenses (both traditional and specialty), and ocular disease, Dr. Polanek joined Advanced EyeCare Professionals in March 2016, and she provides eye care to patients of all ages.

Dr. Polanek, her husband, and four children have made their home in the Beverly neighborhood of Chicago and enjoy being a part of this beautiful community.
