AEP is proud to announce that Dr. Casey L. Hogan  has earned her AAO Diplomate!
Dr. Casey Hogan is the fourth optometrist in the country to attain Diplomate status in Anterior Segment diseases of the eye, from the American Academy of Optometry. What is a Diplomate of the American Academy of Optometry? Diplomates are Fellows of the American Academy of Optometry who have invested hundreds of hours of preparation and assessment in order to be recognized by peers for their extensive skill and expertise in their research or practice.

Jamie N. Hogan, OD, FAAO Jamie N. Hogan, OD, FAAO
Dr. Jamie Hogan grew up in South Suburban Chicago. She graduated from the Illinois College of Optometry and went on to complete a primary care residency at the Dayton, Ohio VA Medical Center.

Dr. Hogan is a Fellow of the American Academy of Optometry and a Diplomate candidate for the Anterior Section of the Academy. Dr. Hogan served as student externship coordinator at the Danville, IL VA Medical center where she held adjunct faculty appointments with four Optometry schools.

She currently practices at Andrew Benjamin VA in Crown Point, IN. She is currently adjunct faculty at ICO and precepts ICO optometry students at ABJ.
Tiffany Polanek, OD Tiffany Polanek, OD
Dr. Tiffany Polanek is excited to be joining THE AEP TEAM in March 2016. Dr. Polanek is originally from a small farming community in southern Minnesota. After completing her Bachelor of Arts degree from Moorhead State University in Minnesota, she moved to Michigan for optometry school. She graduated from the Michigan College of Optometry in 2000. Dr. Polanek then moved to Chicago for continued training at the Illinois Eye Institute, where she completed the Primary Care and Ocular Disease residency program in 2001.

Following her residency, she moved back to Michigan to work in an established private practice, where she focused on primary eye care, ocular disease, and contact lenses. She also worked with student interns from the Michigan College of Optometry as they rotated through her office.

Dr. Polanek's personal life took her back to Chicago, where she has remained since 2003. She returned to the Illinois Eye Institute (IEI) as an Associate Clinical Instructor in 2003, and has remained on staff part-time since then.

At IEI, she provides eye care to patients, and clinical education to optometry students, in the Primary Care Clinic and the Cornea and Contact Lens Center for Clinical Excellence. Dr. Polanek has worked in a variety of private practice locations as well. She has extensive experience in primary eye care, contact lenses, and ocular disease. Stephanie DiGiorgio, OD Stephanie DiGiorgio, OD
Dr. Stephanie DiGiorgio was born and raised in Orland Park, Illinois. She graduated from St. Michael elementary school in 2002 and Carl Sandburg high school in 2006.

She attended the University of Iowa for her undergraduate degree where she studied Health Science and Entrepreneurial Management. She then took her studies to South Florida where she attended Nova Southeastern University College of Optometry in Fort Lauderdale.

Stephanie then completed a double externship in ocular disease and post operative management at the world renowned Bascom Palmer Eye Institute in Miami.

Dr. DiGiorgio began working at AEP in Fall of 2016. She is an active member of the Illinois Optometric Association and the American Optometric Association.
